Capstone Medical is the healthcare professional division of Capstone Financial Services. Originally founded in 2008 as Southwest Medical and Physician Financial Services, LLC — known in the medical community as SMAP FS — the practice evolved alongside the professionals we serve, becoming Capstone Medical in 2012. What began with a group of residents at the University of Arizona Medical Center in Tucson has grown into a nationwide practice, having served clients across all 50 states and several foreign countries.

We understand that the financial journey of a physician is unlike almost any other. Years of training, significant educational debt, delayed income, and then a rapid transition to high earnings — all while navigating complex compensation structures, partnership tracks, and the unique tax considerations that come with a medical career. We've spent more than two decades building expertise in exactly these challenges, and we've built a team of specialists across financial planning and tax to address them comprehensively.

Capstone Medical works primarily with residents, fellows, and practicing physicians at every stage of their career. We also work with nurses and other healthcare professionals who face similar financial planning needs. What we do

Student loan management

Navigating federal loan repayment programs is one of the most consequential financial decisions a physician makes early in their career. We work in close coordination with our tax team to evaluate different filing strategies and their impact on income-driven repayment calculations, helping you make the choice that serves your long-term financial picture, not just the short term.

Long-term disability income insurance

Your ability to practice medicine is your most valuable financial asset. We evaluate both group disability coverage offered through your employer and private individual policies, helping you understand the differences in definitions, benefit periods, and portability so you're protected regardless of where your career takes you.

Sign-on bonus and partnership track planning

The financial decisions surrounding a sign-on bonus or partnership track offer can have significant and lasting tax consequences. We work alongside our tax professionals to help you understand the implications before you sign, not after.

Tax planning and preparation

Taxes aren't a once-a-year event — for physicians, they're something we plan around continuously, not just report on. That includes coordinating filing status decisions (Married Filing Separately vs. Jointly) with student loan repayment strategy, integrating 1099 and K-1 income from moonlighting, locums work, or practice ownership onto the return, and managing multi-state filings for physicians who trained in one state and practice in another. We prepare returns directly for many clients and coordinate closely with outside tax professionals for others, but either way, tax strategy stays connected to the rest of your financial plan year-round.

Retirement plan implementation

We assist with retirement plan setup and strategy on multiple levels: employer-sponsored plans, self-employed retirement structures, and individual retirement accounts. We can also assist qualifying participants in establishing self-directed investment accounts within certain retirement plan structures, providing access to a broader range of investment options beyond what a standard group plan may offer.

Roth IRA conversions and high-income planning

High earners face specific constraints and opportunities in retirement planning that require specialized knowledge. We help physicians understand Roth conversion strategies, backdoor Roth options, and other planning tools designed for professionals at higher income levels.

A practice built on relationships

We started in residency programs because we believed that physicians deserved a financial partner who understood their world from the beginning of their careers, not just after they'd arrived. That philosophy hasn't changed. Many of our clients have been with us since their residency and remain clients today as established practitioners, practice owners, and partners. That kind of long-term relationship is what we're in this business to build.
